I have a switch mode charger that has an output of 12 volts. It is a power adapter for some scales .All good , but i thought they were all 5 volts . Whats the ramifications of plugging my phone into it ?

Powerdelivery 2.0 USB charger bricks can go as high as 20v. They will auto detect what voltage the device needs depending on resistance put between terminals x and y.
